One dollar 2001 - Centenary - Rotated 90 degrees
While striking the 2001 Centenary of Federation (COF) dollar coin there was a problem during the production run of one pair of obverse and reverse dies. The obverse die wasn’t fastened correctly and it slowly rotated as it struck more coins. This meant that instead of the produced coins having the usual medal alignment (obverse and reverse oriented the same way) they were produced with all manner of angles between the obverse and reverse dies. 180 degrees rotations are also known.
